Goal: All young adults with disabilities (ages 12-26) and their families in San Francisco receive the information and support necessary to successfully transition to post-high school options and adult services.
Turning 18 shouldn’t mean falling off a cliff. Yet for too many young adults with disabilities and their families, the transition from school-based services to adult services is exactly that — a confusing shift they’re expected to navigate largely on their own. Families often don’t know what’s changing or what support is available as their child ages, professionals frequently lack visibility into the resources that exist, and coordination between SFUSD, GGRC, and DOR is difficult to achieve.
The Transition Age workgroup brings together families and professionals to focus on helping young adults with disabilities and their families identify, access, and navigate the services available for the transition to post-high school options and adult services — so the path forward is a map, not a mystery.
