Goal: All children with complex medical needs (ages 0-26) and their families in San Francisco can access and coordinate the care they need to ensure well-being for their family and full inclusion in community life.
Children with complex medical needs — and their families — deserve a care system that works with them, not one they have to hold together on their own. Too often, families become the connective tissue between disconnected organizations, each running its own process and system with no shared thread between them. Families don’t know what services exist or how to access them, and even providers frequently don’t know what’s available outside their own organization.
The Complex Medical workgroup brings together families, providers and professionals across the care ecosystem to focus on improving navigation and coordination, so families of children with disabilities and complex medical needs can spend less time as the hub of a fractured system, and more time focused on their child’s well-being.
