Inspired by Collective Impact

The SF Kids Collaborative is guided by the collective impact model — a framework for bringing community members, organizations, and institutions together to drive real, lasting systems change. Rather than working in isolation, collective impact asks everyone at the table to align around a common goal and work together in structured, intentional ways.

The five conditions we aim to follow:

Common agenda

We come together around a shared understanding of the problem and a unified vision for change for children and youth with special healthcare needs in San Francisco.
Shared measurement
Shared measurement
We track progress the same way across all partners, so we can learn together, stay accountable, and know when things are working.
Mutually reinforcing activities

Continuous communication
