We’re committed to improving the lives of families of Children and Youth with Special Health Care Needs (CYSHCN) through collective impact and interagency collaboration.
The SF Kids Collaborative is guided by the collective impact model — a framework for bringing community members, organizations, and institutions together to drive real, lasting systems change. Rather than working in isolation, collective impact asks everyone at the table to align around a common goal and work together in structured, intentional ways.
